Permission Forms

Leaving the School Grounds During Lunch Break

Students are expected to remain on campus (school grounds) during the school hours of 8:35am to 3:10pm, including lunch break. This expectation is supported by provincial policy and PSD Administrative Procedure 310 which indicates students are the responsibility of the principal and school staff during school hours, and 15 minutes before and after. In the case of bus students the procedure applies between the students time of arrival and departure on the bus.

The following exceptions are:

  • Students who attend a school sponsored off-site activity.
  • Grade 9 students who are permitted to leave campus during the lunch break (12:30 – 1:15 pm), and are expected to return prior to the afternoon bell.
  • Grade 5 to 8 students whose parents have submitted a Closed Campus Lunch Permission Form indicating the student can go home for lunch every day.

Changes in Routine

If a student who normally stays at school for lunch needs to leave for any reason the parent/guardian should communicate the details by written note, phone call , or email message to the school office.

Request to Administer Medication

PSD Administrative Procedure 318: Administering Medications/Personal Care to Students outlines all requirements necessary before any school staff member can administer medication to a student.

Before Prescription Medication is Administered

The Request to Administer Medication form must be completed by the parent or guardian, the prescribing physician, and school administration. The school reserves the right to refuse to administer medication.

Prescribed medication must come to school with a parent/guardian in a container labelled by the pharmacy and will be stored in a locked cupboard in the school office. Non-prescription drugs are not to be brought to school.
